  Towards a new mental health information service for sheffield - Questionnaire  

Over the next six months Sheffield Mind will be developing a comprehensive mental health information service for Sheffield. This service will enable people to find out about mental health and social care services for adults in Sheffield, how to access these services and how they help people. The service will also provide information and sources of advice about dealing with mental health problems. Before we embark on this work we need to find out people's views about the existing Smooth Guide booklet and this online directory, as well as the other sources of information that people use. We need to know what works, what doesn't and what people would like to see in the new service.

I would be very grateful if you could spare the time to complete this questionnaire. I am aware that it is quite long and that not all the questions will be relevant to everyone, but please do fill it in. Your views are very important to us in helping develop a useful, up to date, accessible and dynamic mental health information service for the future.

If you have a paper copy of the Smooth Guide it might be helpful to have this with you when answering the questions.

The deadline for submissions will be the 25th April 2008.

Smooth Guide booklet

 
The Smooth Guide booklet is a directory of services for people with mental health needs in Sheffield and mental health professionals. It also contains other, more general, mental health information. The last edition was published in 2005.
